Appointments Appointments are scheduled between 8:30 a.m.
and 5:00 p.m., Monday through Friday. Same day appointments are available for urgent or emergent situations. Despite
our best efforts to work in a timely manner, there are occasions in which
the physicians may be delayed. Emergencies and complex problems may delay
an appointment with a physician. We know your time is valuable and we will
make every effort to provide you with timely service. All HMO and Workers' Compensation appointments require authorization.

As directed by our office please download the forms and bring them completed
to your scheduled office visit. This will help expedite the registration process.

Fees & Payments We make every effort to decrease the cost of your medical
care. Therefore, we request payment arrangements for all office services at
the time they are rendered unless prior arrangements have been made. We accept
cash, checks, MasterCard, Visa, and Discover for your convenience. If we are
a participating provider of your insurance company, we will bill them. However,
payment is the patient's responsibility. We will help in any way we can to assist
you in handling claims.
Insurance We are contracted with multiple insurance entities. Any specific questions
regarding insurance should be referred to our insurance department. Our group
currently accepts Medicare, Cash, PPO, Workers Compensation, and most third
party insurance carriers.
Parking Parking is ample and free. A bus stop is located at Bastanchury and Harbor
just around the corner from our office. Our suite is at ground level and
is easily accessible for the handicapped. Bilingual services are also available.
Prescriptions & Renewals Please request all prescriptions and authorizations for renewals between 8:30am
- 5:00pm, Monday to Friday, when our full medical records are available.
Renewals requested at other times will be filled only for extreme or emergent
circumstances.
